How to create a form template

Learn how to create and customize form templates.

Creating a form template allows you to standardize performance reviews, 1-on-1 evaluations, surveys, or 360 assessments across your organization. With a well-structured template, you can ensure consistent data collection, save time for HR and managers, and make feedback easy to track and analyze. This article will guide you through the steps to create, customize, and manage your form templates.


Creating a new form template

  1. Go to Performance → Campaigns → Form templates, then click Create new template.

  2. Select the type of review for which you want to create a form template.

📌 Note: Once selected, the review format cannot be changed. Make sure you choose the correct format based on your use case. If a mistake is made, you will need to create a new questionnaire with the correct format.

Available formats:

  • 1-on-1 review

    A face-to-face evaluation between an employee and their manager.

    Examples: annual review, professional development review, mid-year review.

  • Private review (less commonly used)

    A confidential evaluation visible only to the manager and administrators.

    Example: collecting information ahead of people reviews.

  • 360° review

    A multi-source evaluation involving several stakeholders (manager, peers, direct reports, etc.).

    Purpose: gather diverse feedback for a well-rounded assessment.

  • Survey

    An internal questionnaire sent to all employees.
    Purpose: to measure engagement, collect feedback, or run internal surveys.

  • Automated 1-on-1 review

    A review automatically triggered based on a defined date.

    Example: an end-of-probation review scheduled 30 days after hiring.


Structure your questionnaire

Once the format is selected, you can structure your questionnaire by adding sections and questions.

Customizable question types

  • Open answer

    Allows free-text responses, ideal for opinions or detailed feedback.

  • Scale

    Enables evaluation using a numeric or descriptive scale (for example, 1 to 10).

  • Choices

    Offers one or multiple predefined answer options (single or multiple choice).

  • Table

    Lets you assess multiple criteria using rows and columns.

Smart modules

Single use questions, designed for specific HR use cases:

  • Review goals

    Review goals from the previous period.

  • Define future goals

    Define goals for the upcoming period.

  • Evaluate skills

    Assess skill levels.

  • Training needs

    Collect employees’ training requests.

  • Evaluation of past training courses

    Gather feedback on completed training sessions.


Reorder questions

  1. Click the three dots next to the form template, then select Edit.

  2. Drag the six-dot handles (two stacked rows) to rearrange questions in your preferred order.


Edit a form template

  1. Click the three dots next to the form template, then select Edit.

  2. Click a question title in the left-hand menu to view and update its settings.

  3. Once all changes are complete, click Update the template in the top-right corner.


Duplicate a form template

  1. Click the three dots next to the form template, then select Copy.

This creates a new form template with the same structure, which you can then edit as needed.


Share a private form template with another user

  1. Click the three dots next to the form template, then select Update accesses.

  2. A side panel will appear on the right. Click + Provide access.

  3. Start typing the user’s name and select them from the list.

  4. After adding all users, click Submit in the bottom-right corner.

The selected users will now have access to view and use the form template.

📌 Note: By default, a form template is set to public. You can, however, choose to make it private either when creating the template or anytime afterward.
